FIT Gurugram – Runner Health Declaration & Consent Form

1. Health Declaration

I certify that I am physically fit, adequately trained, and capable of completing the registered race distance. I understand that participation in the run requires physical exertion.

2. Assumption of Risk

I understand that running is a physically demanding activity and involves inherent risks, including but not limited to dehydration, heat-related illness, falls, collisions with other participants, adverse weather conditions, traffic-related incidents, fatigue, and other medical or physical complications.

I voluntarily choose to participate in the event and accept responsibility for the risks associated with participation.

3. Release of Liability

To the extent permitted by applicable law, I agree not to hold the event organisers, sponsors, partners, volunteers, staff, or authorised representatives responsible for any injury, illness, loss, or damage arising from my participation in the event, except where caused by proven negligence or wilful misconduct.

4. Medical Consent

In the event of an injury, illness, or medical emergency, I authorise the event’s medical team or emergency responders to provide appropriate first aid and emergency medical treatment.

I understand that, where required, I may be transported to a medical facility, and I will be responsible for applicable medical, hospital, ambulance, or emergency transportation expenses.

5. Media Release

I provide permission for photographs, videos, and other recordings taken of me during the event to be used by the event organisers and authorised partners for event documentation, publicity, promotional, and marketing purposes, without additional compensation.

6. Emergency Contact

I agree to provide an emergency contact’s name and phone number for use in case of an accident, injury, illness, or other emergency during the event.

7. Requirements for Participants Under 18

Participants below 18 years of age must have the consent of a parent or legal guardian.

The parent/legal guardian confirms that they:

• Have reviewed and understood these terms and conditions.
• Approve the minor’s participation in the registered FIT Gurugram run.
• Accept the risks associated with the minor’s participation.
• Authorise emergency medical treatment if required.
